Make note of the type of complaint you are filing. You will need to choose one of the following once you enter the Complaint Form:
- Insurance Complaint: A complaint about health insurance, life insurance, property/casualty insurance or auto insurance.
- Vacant Property Complaint: A complaint about a vacant and abandoned home.
- Student Loan Complaint: A complaint involving a student loan, loan consolidation or reduction issue, financial aid, etc.
- Mortgage Complaint: A complaint about a New York State-licensed mortgage banker, broker, loan originator, loan servicer, or mortgage related issue.
- Bank/Lender, or Other Financial Services Complaint: A complaint about a New York state-licensed bank or lender, sales finance company or credit card, and any other complaint type not listed above including Check Casher, Debt Collector/Debt Relief, Bail Bond Agent, Virtual Currency issuer, Rent to Own/Gift Card, Money transmitter, etc.
- Bail: A complaint about a bail agent, insurer that writes bail bonds, or a charitable bail organization.
Note: - Health Insurance Premium Rate Increase: DO NOT File a Complaint. If you are a health insurance policy holder looking to submit a comment about an insurer's application for a rate increase use a Health Insurance Rate Filings Comment Form.
- Healthcare Provider Complaint: A Healthcare Provider submitting a complaint on behalf of a patient must use the Provider Complaint Form.
